PUBLIC REVIEW PERIOD: Monday, August 31 to Monday, September 14
Housing for Health (H4H) is continuing to strengthen and clarify policies, procedures, and guidance for the Housing for Health Partnership (H4HP) System of Care. As part of this work, H4H Division staff are developing new and updated materials to promote consistent, effective, and person-centered practices across the system. Input from partners, providers, people with lived experience, and other stakeholders is an important part of this process.
H4H is currently seeking feedback on two draft documents: the Housing-Focused Outreach Manual and the Complaint and Grievance Policy.
The Housing-Focused Outreach Manual is intended to help create a shared approach to outreach for people experiencing unsheltered homelessness. It outlines how Housing-Focused Outreach can help connect people to Coordinated Entry, shelter, housing, and other services and supports while advancing person-centered, housing-focused practices across the broader H4HP System of Care.

The Complaint and Grievance Policy establishes a clear process for addressing concerns, complaints, and formal grievances. The policy is designed to support transparency, accountability, and person-centered care by helping participants, providers, and community members understand how concerns can be raised and resolved. An accompanying flowchart is also available to help explain the process and guide process steps.
