Transfer Policy & Feedback Form

The Housing for Health (H4H) System of Care Standards establish shared expectations for how the system operates. The Standards are designed to promote consistency across programs, clarify how people access services, and define how housing and resources are prioritized across the system. H4H staff are developing the draft standards in components, with sections being rolled out as the drafts are completed.

When complete, the standards will outline operational policies and procedures for the major components of the H4H System of Care, including prevention, coordinated entry and housing problem solving, outreach, temporary shelter, transitional housing, rapid rehousing, permanent supportive housing, and supportive services dedicated to people experiencing homelessness.

Housing for Health Division staff are currently developing these draft standards and policies to ensure programs operate in alignment with system goals and Continuum of Care (CoC) requirements.

 

CoC members are invited to share their thoughts:

  1. Review the draft Transfer Policy
  2. Fill out the Feedback Form from Thursday, March 5 to Thursday, March 12
