Purpose:

• Create space to explore and troubleshoot short-term and alternative housing strategies for Coordinated Entry (CE) participants not on the queue.
• Identifying and addressing barriers to informal or non-program housing solutions.
• Monitoring progress for participants who remain active in Connectors services but have not yet been housed.
• Ensuring participants with limited formal options receive collaborative, timely support. 

What to Expect: One week prior to the meeting, H4H will send a list of the 5 longest stayers in CE (not on the queue) that we intend to case conference. Connectors should plan to attend these case conferencing meetings if they are actively working with a participant scheduled for discussion or have a participant they wish to discuss. We will have time to discuss additional participants based on Connector requests.

Purpose:

• Resolve barriers for participants with long stays on the Housing Queue
• Monitor and support progress on referrals to housing programs
• Ensure coordination between Connectors, Housing Providers, and CE system staff
• Improve system flow by reviewing cases frequently enough to prevent stagnation and make use of available housing opportunities
• Provide a shared space to clarify participant status, troubleshoot documentation and eligibility issues, and assign timely follow-up actions

What to Expect: We’ll focus on 10–15 queue participants each month, starting with the longest stayers. One week prior to the meeting, H4H will send a participant list out to this group. While we’ll hold the time on everyone’s calendar, attendance is only required if your case is on that month’s list. Connectors without an assigned case are not required to attend, however, your attendance is welcome if you have information about anyone on the list that would be helpful to share.
