Consideration of Privacy and Confidentiality
The Homeless Memorial is an opportunity to honor and remember members of our community who have passed away this year, including people who died while experiencing homelessness, people who experienced homelessness at some point in their lives, and people who worked in our community to support those experiencing homelessness. As we remember their lives and the impact they had on others, we also want to respect the privacy and dignity of each person and their loved ones.
When submitting a name, please be mindful of the privacy of the person being remembered and their family. All names submitted through this form will be read aloud during the memorial.
Before submitting a name, please:
- Obtain permission: Whenever possible, obtain permission from the individual before their death or from an appropriate family member, legal representative, or other person authorized to share the information.
- Protect private information: Only provide the information requested on this form. Do not include medical, health, or other sensitive personal information.
- Consider the impact: Please consider how publicly recognizing someone by name may affect their surviving family members, friends, or others who knew them.
- Respect confidentiality: If you have received information about the individual through your work with a health care provider, social service agency, or other organization that is subject to privacy requirements, make sure you have the appropriate authorization to share that information.
- Honor each person with dignity: Please approach every submission with compassion and respect for the individual and their loved ones.
These guidelines help us create a meaningful memorial while respecting the privacy, dignity, and wishes of those we are honoring and their families.
Submit a Name for the Homeless Memorial
By submitting a name for the annual Homeless Memorial, you acknowledge and agree that:
- You have obtained appropriate permission to submit the individual's name, either from the individual before their death or from an appropriate family member, legal representative, or other authorized person.
- You understand that the individual's name may be publicly read and/or displayed as part of the Homeless Memorial event.
- You understand that you are responsible for ensuring that you have the appropriate permission to submit the name and for addressing any concerns or objections that may arise from your submission or the public reading of the individual's name.
